Providing Functional Assessments to Determine Eligibility for ADA Paratransit Services

Notice is hereby given by the City of Santa Rosa (City) that it will receive sealed proposals for the procurement of the following services:

Furnishing the scheduling call center and staff, qualified assessment staff, an online database of client determinations accessible by the contracting entity, and supervision to determine ADA Paratransit eligibility. For Santa Rosa, the assessments will be held on Mondays and Thursday at the Transit Operations Building in Santa Rosa.

Sealed Proposals will be accepted until 3:00 PM on May 3, 2012 at Santa Rosa City Hall and will be addressed to:

2010 OUTSTANDING PARATRANSIT PROGRAM AWARD!!

 

Santa Rosa Paratransit

The City contracts with MV Transportation to provide dial-a-ride (i.e. door to door) transportation services within the Santa Rosa city limits and the unincorporated Roseland area. To be eligible, the disability must prevent you from using fixed route transit (Santa Rosa CityBus or other transit systems).

To be certified or re-certified as eligible to ride Santa Rosa Paratransit:

  • Call (707) 541-7180 or TDD (707) 541-7184 to request an application and to schedule an in-person interview.
  • Bring the application to your interview. We can help you complete the application at that time.
  • Information you provide from your health care provider will be considered, but is not required.
  • At your interview you will be asked to assess your ability to use Santa Rosa CityBus fixed route service.
  • If necessary, a functional assessment will be conducted.

Functional Assessment

  • A Functional Assessment is used to determine your physical and congitive ability to use Santa Rosa CityBus fixed route service and under what conditions you would be eligible to use Paratransit services.
  • The Assessment includes:
    • A simulated trip to and from the bus
    • Boarding the bus
    • Negotiating a curb and a curb cut
    • Crossing a street
  • The Assessment evaluates:
    • Balance
    • Strength
    • Coordination
    • Range of motion

Determining Eligibility

  • Your eligibility to use Santa Rosa Paratransit service is based on the information you provide; on your application, at your interview and if necessary, during your functional assessment.
  • Interviews and functional assessments are conducted by an independent organization - C.A.R.E. Evaluators.
  • Variables in the environment and their effect on your ability to use Santa Rosa CityBus will be considered.
  • You can provide the names and the telephone numbers of two professionals who are familiar with your disability or functional ability. Santa Rosa Paratransit may contact them if additional information is needed.
  • You eligibility to use Santa Rosa Paratransit may be conditional, which means that you may be eligible to use the service under specific conditions.

 Appeals Process

  • If you are not satisfied with your Santa Rosa Paratransit elgibility determination, complete the apeal form included with your determination letter and return it to the address below within 65 days.

 Oakmont Shuttle

Beginning October 1, 2008

Santa Rosa CityBus Route 16, the Oakmont Shuttle, will pick up Oakmont residents who are ADA Paratransit Certified at their homes!

ADA Paratransit Certified Oakmont residents can enjoy this free shuttle to pick them up at home and take them to and from the activities they enjoy within Oakmont.

And they can go along on the daily shopping trips to:
  Mondays, Wednesdays and Fridays - St. Francis Center (Safeway)
  Tuesdays - Montgomery Village
  Wednesdays - Montecito Shopping Center (Oliver’s Market)
  Thursdays - Long’s Drug Store (Farmer’s Lane)

Just call MV Transportation to request a ride, 546-1999. This is the same phone number to request Santa Rosa Paratransit rides.  Make reservations up to 7 days in advance or between 8:00 and 10:00 a.m. on the same day as the ride.
Reservations will be scheduled on a first-come, first-served basis, so call in advance to ensure your place on the bus!
We’ll pick you up anywhere within Oakmont and give you a ride home, too! Just let us know when you call!

How does this work?
This is the same Santa Rosa CityBus Route 16 bus that already serves Oakmont. What’s new:  as the bus travels along its regular route it will simply make small detours to pick up ADA Paratransit Certified Oakmont residents at their homes, and then return to the regular route.

Classes now forming!
Santa Rosa CityBus is scheduling training
classes to introduce ADA Paratransit Certified residents of Oakmont to the convenience and benefits of taking the Oakmont Shuttle to activities, services, shops, even to visit friends within Oakmont and on the weekday shopping trips.
