OPEN ENROLLMENT FOR ALL BENEFITS WILL BE

OCTOBER 10, 2011 - NOVEMBER 4, 2011

You should have already received open enrollment packets from CalPERS.  Make sure to check your Health Plan Statement (even if you do not plan to make changes) to confirm information is correct and check for any alerts.  If you haven't received a packet, please go to the CalPERS Open Enrollment website at www.calpers.ca.gov.

ALL CHANGES ARE EFFECTIVE JANUARY 1, 2012

Open enrollment is your opportunity to make changes to your benefits plans or add additional eligible family members to your plan. For additional questions about open enrollment or your benefits, please contact Noreen Griffin at benefits@srcity.org or at 543-3062.

ALERT: CalPERS is requiring us to capure social security numbers for all family members every time there is Health Change Form Submitted. When completing your enrollment form, please make sure that all social security numbers are filled in for all family members.

HEALTH PLAN CHOOSER 

CalPERS has a web based tool, the Health Plan Chooser, that allows members to determine which CalPERS plan best suits their needs. The tool provides members with the means to compare health plans using:

  • Plan costs
  • Plan features
  • Plan performance measures
  • Available doctors by plan
  • Covered services

Dependents Aged 26 & Under! CalPERS medical coverage continues to be extended to children up to age 26. Subscribers may enroll and re-enroll adult children during the annual CalPERS Health Open Enrollment period for the new plan year beginning January 1, 2012.

 DECLINING HEALTH COVERAGE

Safety employees wishing to waive health coverage because they have other coverage may do so at any time. Please complete the appropriate form along with the CalPERS Health Coverage Form and return to Human Resources at City Hall, Room 1. 

Police Waiver of Health Insurance-You may choose to receive an additional taxed $200 per month or the current allotted amount for dependent care. Please note that in order to receive the allotted amount you must enroll in the Dependent Care Plan for 2012.

Fire Waiver of Health Insurance Form-You may receive the current allotted amount for dependent care. Please note that in order to receive the allotted amount you must enroll in the Dependent Care Plan for 2012.

 LIFE INSURANCE

Although employees may apply for supplemental life insurance at any time, open enrollment is an excellent time to assess your current supplemental coverage levels. Please note that all units have a City paid basic life insurance policy. Amounts are are follows:

  • Unit 2 (Fire)-$12,000
  • Unit 5 (Police)-$10,000
  • Unit 9 (Safety Management)-$30,000

Employees may apply for up to $200,000 of supplemental life insurance for them and $50,000 for their spouse. If you would like to view supplemental life insurance rates, please click HERE
